I always wondered about that. When I biked in NYC the huge safety advantages were the lower speed diferential with traffic and that most cars on the road (cabs and commerical drivers) were used to dealing with bikes. Not to say they were friendly; but they're probably not going to kill you. Now, you're much more likely to hit an pedestrian that steps in front of you, or have someone open a car door on you, or have some other smaller accident, but what scares me about riding here in NC is that cars aren't used to having bikes around and that they're going much, much faster than me. In NYC, I might get hurt, but in NC I think I'd be killed if I was hit by a car. I think I heard on the radio in NYC once that in the past year not one biker who was wearing a helmet had been killed. That's not too bad.
