Just be honest about what you thought of her as an employee. If you don't, then you're lying for her to her prospective employer, which is unethical. Who knows why she used you as a reference? Maybe she's crazy, maybe she actually got along worse with everyone else. In my experience, references are definitely for more than just finding out employment dates.
