Even when traffic is crawling the cars on the road you're entering are usually not evenly spaced. Someone usually is not crowding the guy ahead of them so there is space to ease in, especially over half a mile. It's all about finesse.
I look ahead as I drive down a lane like that, with my left turn signal on. That way I'm at least showing some intent (FWIW!). I try to get as far down the lane as I can, but when I see a good space I move into it, slowly so as not to disrupt the flow too much or to piss off the guy I'm moving in front of. If someone wants to blast past me and cut in at the head of the lane, that's fine. They'll be a few seconds ahead of me. And in heavy traffic, who cares?
On average this works about as well as running to the end of the lane and trying to force one's way in.
I hit a spot like this coming north on I-91 into Harford, CT when I need to get onto I-84 heading toward Boston. There's only one lane to 84 and it often bogs down over 1/3 mile before the exit. I run up to about 100 yards or so from the exit and ease into the flow. Works fine and I've never had anyone get pissed.
