I don't know about Dell, but most companies put some cost responsibility on the owner of a product. The only exception I know of is Apple's AppleCare warranty which also covers shipping.
For cars, the dealers all know they have to do some diagnostics; it comes with the territory. Some percentage of that time is built into their base labor rate as overhead. The rest comes out of the repair cost, which either customers pay or in the case of warranty work, the manufacturer pays. No matter who pays for it, dealers typically do not directly recover all the diagnostic costs. Some of it is a cost of doing business.
Whether you think it is fair for any party involved is immaterial. It's the way the business works. The exact policy for diagnostic time varies by dealer and manufacturer, but it works basically the same way, and has for a long time.
Take dealers out of the loop and go to an indy. They will still charge for at least some diagnostic time, and most build some of that into their hourly rate. If they have to do a lot of diagnosis, they bill that separately and are typically upfront about what that rate is.
