The "typical" house won't use conduit for speaker or A/V wiring.
As DDon said, you just cut a hole and fish the wire through. The easiest way for that is to drop a small-linked chain down from the top hole, use another piece of wire to grab it from below.
There are plenty of recessed wall boxes designed for data/communication use that are open in the back.

if running speaker wire in a concealed space, you should use plenum-rated wiring, not typical lamp-cord stuff
