We went to a local kitchen/bath place for the cabinets, which are semi-custom. Prices at these places vary widely, so go to more than one. Cabinets can break the budget so shop carefully. However, don't ignore a local cabinetmaker. Sometimes these guys can give you something really great for not much more than you'd pay for semi-custom.
We did mid-range appliances: cooktop, dishwasher, fridge, double wall ovens. I'm not sure there is a lot of value in the higher priced appliances on a day-in day-out basis. They have some nice features, but can get very pricey. I think we did all our appliances from Sears (GE, Kenmore, one other). Prices were better than BestBuy for equivalent stuff. However, if there is a local appliance place they may be able to give some good pricing. The reefer is a big part of the total appliance cost. Decide what you really need/want and what you can live without. Fro example, an icemaker is a given in a fridge, but ice and water through the door can be a high-ticket option. I do recommend a bottom freezer. Once you use one of these you can't go back to a top or side freezer.
We had the kitchen place act as the contractor. They had a two-person crew come in and move a couple of walls, change a doorway, change the window to a bump-out, add a peninsula, install cabinets and countertops, put in a new ceiling and lights, etc. They brought in the plumber and electrician. Then a flooring guy came in and did the oak floor.
I think the whole thing was around $30K and took 6 weeks. We could have done the work ourselves, but it would have taken a lot longer and there would have been lost income for the time it took to do the work (I'm self-employed) so paying someone was definitely the way to go. You can spend a bundle on a kitchen and it pays to get it done right, but shop for prices and get references if you aren't going to DIY. You may also be able to find a contractor who'll work with you, which can save some bucks but still deliver a top-quality job.
