the main issue with a car engine is the range within which it must be designed to operate. Or more precise: the rapid load changes which it meets and its response it has to provide it terms of torque (power) output and emissions.
So one shouldn't compare stationary engines (even those on the ships or on railroad locomotives; I'm leaving really big engines - motor units - deep aside) to ones they put into cars. They share different designs and running conditions.
