Wow. Very unfortunate situation. I just can't see why you wouldn't put the car in neutral and let it rev itself to death. Obviously it's very easy to say that while sitting at a computer; I guess in the heat of the moment you tend to lose sight of some obvious steps. Probably a bit of tunnel vision - the driver was probably focused on keeping the car on the road, avoiding traffic all while standing on the brakes.
I hadn't thought about this downside to keyless ignitions as well. Three seconds in this type of situation is probably an eternity. You can flip a key off in a fraction of a second and get back to steering the car out of harms way but to hold down a button for three seconds would be almost impossible in heavy traffic or poor weather conditions.
