A call to Delta (got a live person almost immediately!) and a five minute conversation left me with the model number (#602) and a repair parts kit (RP4993). There is no guarantee the repair parts will solve the problem as wear and corrosion can cause damage that isn't fixable with a kit, but the kit is $5 and widely available, so it's the obvious first step.

Thanks everyone for the help - hopefully I can completely avoid cutting a hole in my way. :D
