I recall that the anti scald valve in some faucets had a blue/red piece of plastic that set the mix of hot and cold incoming, and the valve adjusted it for any surges. If you have an anti-scald shower, maybe yours needs an adjustment. the fact that the sink works hints at this.
My brother built an addition on his house and installed a tankless heater in the new bath as it was so far from the hot water tank. Shower and sink worked fine. But I noticed that when I washed my car at his house, I had hot water at the hose outside. The plumber had capped the hot line and split the cold line into both the tank and the cold side of the faucet, but that line ran on further through the wall to the outside faucet. So turning on the outside faucet triggered the heater. It's still that way years later.
Maybe yours is plumbed incorrectly?
