difficult co-workers...advice?

Let's say you had a co-worker you didn't get along with. Let's say also that you work closely with this person in a small department at a small company, so it's not feasible to just avoid the person. Now, let's say you and this person share a manager who is a very cautious, circumspect person who is (admirably) unwilling to take sides and wants to weigh both people's versions of the story equally.
The difficult co-worker calls you a bad name and behaves unprofessionally. The first time this happens, the manager insists to you that there must have been something you did to provoke this reaction, and asks you to examine your own behavior. Then it happens again, but you let that one go. Then it happens AGAIN. You see a pattern. But your manager refuses to see it from your perspective. It totally reminds me of a Simpsons on TV the other night where Marge befriends Bart's bully and refuses to listen when Bart tries to tell her that this kid is a sadist.
I've worked here for almost eight years, longer than the manager and much longer than the coworker. It's a great, secure job. But how many more times would YOU tolerate being insulted to your face in your workplace, and having your manager do nothing about it except feed you lines about "Problems are never caused by just on party, can you think of anything you did to cause this?", before you picked up and moved on?
