Of course there's about 14 inches of that small coolant pipe that you can only visualize by removing it from inside the PCV hose and one small hole would be enough to cause trouble. If you don't want to damage that PCV hose for a visual you could pressure test the pipe.
If the AMM signal is pre-empted by the fan running that suggests either a ground fault or a wiring defect. Make sure your grounds at the lifting lug (near the thermostat) and the ground wire on the front of the transmission are all secure.
We may be way past this but make sure that the connector on the APC box is securely attached and the ECU connector as well. I was diagnosing a customer car one time and when I lifted the APC box up to disconnect the wiring the connector just fell off. Someone had failed to fully attach it. Easy fix on that car!
