Timing chain

I have a '91 9000 Carlsson 2.0T 180k kms and (I think) the timing chain is
making a rattling noise (noise is from the left side of the motor near the
auxiliary belts) upon start up but quiets after warm up. Could someone
please confirm with me that the chain needs replacing and tell me is the
job really involved? Instructions on what to do and take off would be much
appreciated. I am told it is a 4 hour mechanic's job (meaning 8 days for
me (semi mechanically competent ).

Also is there a scheduled mileage that the chain gets changed as
preventative medicine.

I have been using Castrol 10W40 in the car, would anyone recommend
switching over to Shell synthetic 5W50. Is this detrimental to a well worn
motor.
