Do not blame your insurance company with regards to salvage title. In
general they do not care, however your state most likely has a law in
place that says something like this, if the repair costs = or exceed 80%
of the value of the vehicle the title must be turned in and a salvage
title will be issued.  I have seen the number as low as 75% of value. If
the vehicle is to be destroyed (or parted out) the title shall be
surrendered for destruction. In other states they have laws which in
essence say that if repair costs plus salvage value is > or = the value
of the vehicle a salvage title must be issued.
