inside Fogging

All of a sudden a couple of months ago the windshield started fogging up.
No jokes about being in Canada please, it was summer at the time ;) but
this has become more of an annoyance now.

After about 10 minutes' driving I'll have to flip the ACC back to defog to
clear the inside of the windshield. And I'll have to keep it there most of
the drive to work or the car will fog up again. The rear window also gets
fogged by the time I get to work (30 mins).

There are no apparent leaks in the cooling system, the blower is new and
operating fine, the coolant is high, there is no smell of coolant in the
cabin.

After sitting for a while in the parking lot, there will be a big fog spot
on the inside of the window in front of the driver's seat.

The passenger's side is fine.

Would a pin-hole in the coolant pipe cause this, but go otherwise
undetected?
