Balancer Shaft Slippage

My '95 900S is in the garage at the moment with the engine on the bench in
pieces attempting to fix a vibration problem. The problem was located to
the exhaust side balancer shaft being exactly 180 degrees out of alignment,
which was causing one hell of a shake.

What we are stuggling to explain is how this could have happened, all the
gears and chains seem OK, the tensioner looked fine (all have been replaced
though) - has anyone else seen this kind of fault? I am worried that if we
reassemble it without finding the cause it will just happen again, and this
engine strip has cost enough already thankyou!

BTW - It has 98K miles, so its not very old.
