Balance shaft transmssions DO suffer wear, usually sprockets and the
tensioner. When things happen, the chains
'jumps' on the cogs of the inlet side balance shaft resulting in the rattle
around 2000 - 2500 rpm. It can also be noisy.
On B204i engines (2 litre naturally aspirated) I deactivate the balance
shaft system with taking the chain out and
blocking the oil passage to the tensioner, making the engine similar to
B206i (Saab's 'balance less' version of the B204i).
On other versions, just replace the whole system with new parts.
Good luck
