Balance Shaft Chain

I drive a -92 9kCS with the 2.3 balance shaft engine. The balance shaft
chain has caused trouble frequently. At 80kkm the engine has been rebuilt
due to a broken timing chain, I assume the cause was a balance shaft chain
that had snapped and caused the timing chain to snap as well.
I bought the car at 165kkm. I was concerned with the engine vibration and
noise and ended up replacing the timing and balance shaft chains along with
the sprockets and guides at 180 kkm.
Some time later on a frosty morning I heard a brief noise that reminded of
the balance shaft chain skipping some teeth, today morning I noticed the
same noise again.
I have started to suspect the balance shaft chain tensioner. Is this
problem familiar to anyone?
