Windshield manufacture is a kind of specialized business - First flat plate
glass is made and cut to size, then heated in some kind of mold or fixture
so that it bends to shape. Two of those bent pieces are placed against
each other with a layer of plastic between them. The joined pieces are
racked up in a long fixture and rolled into an autoclave, where the air
inside is heated and compressed to something in the neighborhood of 400F
and 100 or 120 psi. The heat and pressure bond the glass to the plastic
liner. A defective windshield such as you have received is unusual, but
given the complexity of the manufacturing process, it seems reasonable that
some bad ones will get made.
